1- I signed a medical records release form? Did you receive my records?

We are still in the process of receiving medical records. Please call the office and ask one of the staff members to verify if they were received.
2- I need a medication refill from a provider at CFP who now practices at
your clinic. How can I get a refill?

​If you have not yet established care with our practice, please fill the form above. We will send you an invite to join the patient portal--you will be able to fill out consent forms there. After you have completed this, you can call our office and request a refill. Due to the high volume of emails we are currently receiving, it is best for you to contact the office via phone in order to expedite the process. Please note that your provider may require an initial visit at the practice in order to refill your meds.
